Rates and Accepted Insurances

Your copayment will depend on your individual insurance plan. Please contact your insurance provider to obtain an estimate. Typical copayments range from $12 to $25 per visit, but may vary significantly depending on your specific benefits and deductible.

Private Pay Policy (If not using insurance):

Payment in full is required 24 hours prior to the start of each visit. If you would like to seek reimbursement from your insurance, a superbill can be provided at no additional cost. Please note that reimbursement is not guaranteed and will depend on your insurance provider’s policies.

$310 for initial visit (45 to 60 minute intake visit)

$155 for follow-up visits (20 to 30 minutes)

Administrative Fees for Completion of Forms:​

This practice focuses on providing outpatient psychiatric evaluation and medication management. Participation in the completion of employment-related, disability, housing, or legal paperwork is outside the scope of services provided by this practice. This includes, but is not limited to, forms related to the Americans with Disabilities Act (ADA), Workers’ Compensation (WC), Family and Medical Leave Act (FMLA), disability claims, Emotional Support Animal (ESA) letters or related documentation, legal proceedings, or other employment or housing determinations.

Simple excuse notes confirming that a patient attended an appointment may be provided at no charge for purposes such as missed work or school.

Requests for completion of forms or documentation beyond a simple visit verification note may incur a fee of $75 per form if the request falls within the scope of services provided by the practice. These charges are typically not covered by insurance and are the patient’s responsibility.
